I am not a true diesel mechanic, so you should consult a good one, if you have not already done so. They are worth their weight in gold.
From what I understand, there are many causes for this, including a crack in the DPF. Check the ceramic monolith for damage. Cheap oils (non CJ4) can damage the coating on the monolith. Light duty driving can prevent the system from working properly in the first place.
So long story short, consult a good diesel mechanic and get your truck on the road as fast as is reasonably possible. Good luck.
